Purpose of this study: This questionnaire, as part of an academic research study, is intended to collect relevant and valuable data from parents or guardians of enrolled students. It examines how school management strategies, and the involvement of parents at home, relate to smartphone distraction and student learning outcomes.

Instructions: The questionnaire takes about 8 to 10 minutes. There are no right or wrong answers — please answer based on your own experience as a parent/guardian. Questions marked with a red asterisk (*) are required.

Privacy notice: Participation is voluntary, and thank you for your willingness to participate. Your name, your child's name, and contact details are not required. The system uses an automatic anonymous participant ID for research verification and possible withdrawal requests. Your responses are confidential and will only be reported in combined, anonymised form.

Note: this instrument is a supplementary source of parent perspective for context. The study's primary quantitative and qualitative analysis is based on the student questionnaire and the school leaders/teachers questionnaire.
